Retention Alert is for reporting students struggling with personal challenges which are likely to interfere with their ability to perform academically. A Retention Alert should also be submitted for students needing help to make good decisions about their class registration due to personal challenges. The form is located in Self-Service, navigate to the Retention Alert tab, search for a student using their student ID number and choose the Contribute Retention Info tab. Submitting this form will prompt outreach to the student by a CARE team member. The CARE team consists of student services staff trained to offer support, guidance and resources when students are faced with personal challenges that interfere with their classroom performance.

The General Student Report Form is for reporting students exhibiting odd, unusual, or bizarre behavior or whose behavior may violate HCC’s Code of Student Conduct (for example - disrespect toward employees, inappropriate language, theft/vandalism, unauthorized entry, illegal use of drugs/alcohol, disorderly/lewd/indecent, abusive, sexually suggestive or harassing, disruptive, providing false information). Also used to report student behaviors threatening to self (suicidality) or others (aggression, threats). This online form is found under the Faculty/Staff tab of the HCC website, in Popular Links. Reports go directly to the Dean of Students Office for response by the Dean of Students or a Behavior Intervention Team (BIT) member.

The Academic Integrity Reporting Form is for making reports related to academic dishonesty such as cheating, plagiarism, or misuse of College computers. Faculty submit this form as part of Step 1 (Instructor/Student Communication) within the Academic Integrity Violation Process (see Student Handbook for details). Reports go directly to the Vice-President for Academic Affairs & Student Services for response. The Dean of Students Office monitors when Academic Integrity Report Forms are submitted for students with a concerning history of prior Code of Student Conduct violations and shares this information with the VPAASS.
